we've put up this form in an attempt to cut down on the junk mail streaming into our mailbox. the name, email and message fields are required, all the others are optional, but helpful. if you'd like to send a file, hit "browse..." and you'll get the "open file" dialog box which lets you pick a file on your computer to send. then hit "send" and you'll go back to the table of contents page (toc.html).

of course, please let us know if you have any problems with the form, and rest assured that we will never give out your name or put you on any kind of a mailing list; all correspondence remains just that.
